Long Term Care

We help people understand the various long-term care options available to them.  We help people apply for programs and benefits, and serve as the access point for publicly funded long-term care.

The ADRC provides:

  • Information about the choices you have when making decisions about where to live, what kind of help you need, where to receive that care and help, and how to pay for it.
  • One-on-one consultation to help you think through the pros and cons of the various options in light of your situation, values, resources, and preferences.
  • Information about Wisconsin’s long-term care programs and help connect you to resources in your area.
  • The ADRC can complete an assessment to determine if you will be eligible for public funding for your long-term care.
  • The ADRC can help you prepare your Medicaid application, if eligible.

Access to Publicly-Funded Long-Term Care Programs

For people who request it, the Aging and Disability Resource Center will administer the Long-Term Care Functional Screen to assess the individual's level of need for services and eligibility for long-term care benefits. Once the individual's level of need is determined, the Aging and Disability Resource Center will provide information about the options available to him or her, to stay in the Medicaid fee-for-service system (if eligible), or to privately pay for these services.
